This sample notice can be used for notifying residents and families when residents and/or staff members at your facility have tested positive or received a probable diagnosis for COVID-19. In drafting the notice, we have attempted to not only let people know that a positive case has unfortunately developed at the facility, but also to establish a broader context.
You may want to edit the notice and provide it to your residents and sponsors/guardians in writing, or you may want to modify it to serve as a call script and let individuals know of your positive or probable case “in person.”
